Changes to online prescription ordering

The current online medication request form will only be active until the end of December 2019. It will be replaced by a new system called Patient Access, which links securely with your prescription data from our practice.
Patients of the former Cityview Practice who were already registered for Patient Access will need to register again. We apologise for any inconvenience.
